This is what we have now - in capitated plans physicians get paid for annual participation , and in fee-for -service HMO's we get paid a set fee per billing code determined by the insurer.
Only in out-of pocket "no insurance accepted" practices physicians set their own fees.

As I suspected, Gera posted not direct quotes, but his ad lib interpretations.

I think only doctors who don't have their own practice can think of it as a good thing..every doctor who has own business know they won't be able to survive if all their reimbursements would come from Medicaid (it's so ridicously low)...

btw, about the infant mortality rate in US.
an ObGyn i talked to told me that outside US they consider really low birth weight infant "un-salvageable" and they don't count as "infant mortality" statistics. While in US every baby delivered counts towards the mortality stats, even those below 500gram birth weight, where mortality rate is close to 90%.
If you do apples-apples comparision US infant mortality rate is about the same as Sweden or Norway

Blacks (who are almost 13% of the population) have infant moratlity rate 2.5-3 times higher than whites...
when they compared whites with college-educated blacks (meaning the external risk factors -young age, drugs, poor pre-natal care, etc. didn't play a role) the difference was STILL 2 times higher that of whites...that means a lot of it is genetic...

Whowants their wages set by the government. And do not tell me we have italready... I know - I work for GM ... However - I do not want thegovernment set my wages.
Who sets theirs? Ah, but they vote for their wage increases all the time.

Mike, I know... It does not go by weight, it goes primarily by gestational age, and it is a moving target. His number might be a little old, since at 23 weeks and 6 days and at weights of less then 440 mortality is about 50-50.:-) It goes up from there and depends on a large number of variables. This is a huge very specialized discussion, btw, so I'm not sure this is the right place for it, but I can talk about it for hours:-) . In my practice there is a number of infants who were born barely over 1 lb at 23and1/2 weeks and they are healthy. BTW, most of those babies get some version of Medicaid to supplement neonatal ICU care where they spend 4 month at 15K per day. We all apy for them anyway.

I thought the point of discussing the mortality rate was not about "who pays for what" but to show that the US is somehow behind other countries in overall mortality and infant mortality...someone wanted to make a point that our system is not the best...*-)

Sveta, if you want a separate topic on infant mortality, I am game!
But I'm afraid it will bore people to death, since there is a lot of statistic there starting with maternal rates of Group B strep colonization and so on.....:-)

:-) No, I don't want a topic on infant mortality especially on Strep B colonization :-) but the point is it is decieving to look just at numbers (those numbers are collected differently, the populations are different, the genetic factors are different), and just because we are behind SOME countries, still doesn't mean that our system is not better then MOST socialized systems...
